In assessing a patient for physical therapy, gathering history means building a broad view of the person’s health, lives, and daily functioning so you can interpret the current problem and plan care effectively. The history should cover activities and participation to understand how the patient functions in daily life, which areas are limited, and what participation aims exist. It also includes family history to identify potential inherited or familial factors, current conditions and general health status to capture ongoing issues and other health risks, general demographics to provide context such as age and living situation, and growth and development patterns to assess age-appropriate milestones and needs, especially in pediatrics. This wide range of information helps you see the patient as a whole, rather than focusing on a single data point like medications, imaging, or goals, which belong to other parts of the evaluation and treatment process.
